If you have been in a car accident, you may need to obtain a copy of the police report. How you obtain that police report (or accident report) will vary depending on where the accident occurred. Since we represent many car accident victims on Long Island, we thought it might be helpful to share information on how to obtain a police accident report form the various police departments on Long Island.
Obtaining a Police Accident Report in Nassau County
Nassau County Police Department reports can be obtained by sending a request to Nassau County Police Department, Legal Bureau; 1490 Franklin Avenue; Mineola, New York 11501. You must include a completed “Request for Records” form and a self-addressed stamped envelope. Here is a link to the Nassau County Police Department website where you can obtain the appropriate form: http://www.police.co.nassau.ny.us/pdf/dept_forms/pdcn151.pdf. If you are requesting a report for someone else, you must also include an authorization from the person involved in the incident.
Obtaining a Police Accident Report in Suffolk County
Suffolk County Police Department reports can be obtained by sending a request to Suffolk County Police Department, Central Records Section; 30 Yaphank Avenue; Yaphank, New York 11980-9705. You must include a completed “Application for Public Access to Records” form, a check for $1.00 and a self-addressed stamped envelope. Here is a link to the Suffolk County Police Department website where you can obtain the appropriate form: http://apps.suffolkcountyny.gov/police/documents/PDCS-5414e.pdf. If you are requesting a report for someone else, you must also include an authorization from the person involved in the incident.

Obtaining a Police Accident Report from Local Police Departments on Long Island
Long Island has many incorporated villages, two cities and towns that have their own police departments. Sometimes, these local police forces respond to accidents and prepare the accident reports. In Nassau County, the City of Long Beach and the City of Glen Cove have their own police departments. The Long Beach Police Department is located at One West Chester Street, Long Beach, NY 11561. The Glen Cove Police Department is located at 1 Bridge St. Glen Cove, NY 11542.
Each of the townships in the eastern end of Suffolk County has their own police department:
Riverhead Police Department, 200 Howell Avenue, Riverhead, NY 11901
- Southold Police Department, 41405 Route 25, Peconic, NY 11958
- Southampton Police Department, 110 Old Riverhead Road, Hampton Bays, NY 11946
- East Hampton Police Department, 131 Wainscott Northwest Road, Wainscott, NY 11975
If you need a report from one of these police departments, you must request it directly from the department involved.
